The Home Support Program at the Almage Senior Community Centre is open to English-speaking seniors experiencing a loss of autonomy. The program is open to anyone 65 years of age and over and living in the below mentioned territory.
Nous sommes le seul centre communautaire anglophone pour personnes âgées dans l'Est de Montréal. Nos services sont offerts dans les neuf (9) arrondissements énumérés ci-dessous :
Aider les personnes âgées à améliorer leur qualité de vie, à encourager l'indépendance et l'autonomie,
et à prévenir le placement prématuré en institution.
The services available in the program include the following;
Our professional staff will visit our isolated seniors on a regular basis to evaluate their health progress and communicate with
family members if necessary.
Regular calls to ensure all our members are keeping well and if they require any services from Almage Senior Community Centre.
Follow ups with family members concerning their loved ones and/or with individual members.
Ensuring that our members receive the proper information when navigating the Social & Health care networks for their medical needs.
Regular calls to members who are facing difficulties.
Every member who cannot take public transportation and is not entitled to adapted transport can receive taxi coupons twice (2x)/week to participate for various activities for a nominal fee.
Un repas sain comprend un fruit, un pudding/yogourt et un dessert. Chaque repas coûte 8 $, avec une commande minimale de 3 repas. Par conséquent, 24 $ pour le retrait des repas au Centre ou 27 $ pour la livraison à domicile.
Every member is entitled to purchase taxi coupons at a reduced rate of 50% for all medical appointments. The Centre will pay the remaining 50% of the cost.
Le centre favorise l'approche « entre aînés, on s'entraide » dans les services qu'il fournit. Almage a pour but d'améliorer la qualité de vie des aînés vivant de façon autonome.
Ces programmes et services visent à valoriser la dignité de la personne aînée, à soutenir son indépendance et à l'inciter à s'investir au sein de sa communauté.
